Personalised Care Plans
We tailor your care, housekeeping, dietry requirement and activities to suit you and they way you like it. In order to make sure that your care is centred around you, we carry out a comprehensive assessment of your needs, likes and preferences. This assesment and ongoing care discussions with one of our Care Co-ordinators formulate your 'Personalised Care Plan' and this is how we make sure you receive your care the way you want it.
Pre-assessment
Before coming to stay at The Old Vicarage, we will visit you at home or in Hospital to talk to you about your upcoming stay. We will ask you questions so that we can get to know you well, such as:

Your Plan of Care
Every resident who stays with us for a longer period will have an ongoing individual care needs assessment, so that together we can decide on a detailed plan of care which is absolutely suited to your specific requirements. You may wish to have a family member or friend along to your reviews to help you make decisions on your plan of care. All of our residents have individual needs and to ensure we provide 'holistic' care (care that looks at the whole person, not just their personal care needs) we will talk with you regularly about how you like things to be. This will include questions such as:
- What helps you to feel safe?
- Do you need any assistance with communication?
- What medical needs do you have? Do you require medication?
- What food and drink do you enjoy?
- Do you have any continence issues we can support you with?
- Do you need assistance with your teeth or dentures?
- How do you manage with washing, dressing, showering or bathing?
- How is your sleep and do you need any help at night-time?
- How is your mobility? Do you need assistance with this?
- Do you suffer with any problems such as anxiety, low mood or memory loss?
- Do you have a religion or cultural requirements to arrange?
- How do you like to spend your day?

When assessing individual care needs we will not compromise on respecting dignity, individuality and personal opinions. We make sure that decisions are made 'by' our residents, not 'for' them, wherever possible. At The Old Vicarage, we value diversity and ensure inclusion. We make sure that people are looked after in the way that they wish, regardless of age, disability, gender, race, culture or sexual orientation. We ensure that our staff are trained in non-discriminatory practice and that we are able to protect people from prejudiced or institutionalised practices. We ensure that our care looks at the ‘whole’ person, their likes, dislikes, personality and beliefs.
